If you wish to register a family member to the Wellness Check service, please read the information included on this page prior to filling out and submitting a Wellness Check Application!
There are No Requirements to receive Wellness Checks for your family member(s). This service is at no cost to community members of Fort William First Nation.
Community members who are included in the Wellness Check service must submit a new application annually to ensure their enrollment into the service.

Below is the application for the Wellness Check service. Instructions on where to submit the application are included on the final page of the application. The application may be filled out online and submitted via email or submitted in person!
